All concerned Thomasian students, administrators, faculty, and staff who have not yet received their booster doses are strongly encouraged to register and participate in the PINASLAKAS Nationwide COVID-19 Booster Vaccination Campaign. Kindly take note of the following details:
Date: September 22, 2022 (Thursday)
Time: 08:00AM to 05:00PM
Venue: UST Practice Gym
Registration:
A. University Staff & Students
- Fill out the COVID-19 Booster Dose reservation form via thomedss.ust.edu.ph.
Deadline is on September 19, 2022 (Monday).
- Register in the manilacovid19vaccine.ph
B. Direct-hired staff & University staff dependents
- Send an email to healthservice@ust.edu.ph using the subject heading “COVID-19 BOOSTER VACCINATION.”
- Register in the manilacovid19vaccine.ph
Kindly bring the proof of registration on the day of vaccination and observe the following Health Protocols: 1. Wear well-fitted facemask.
2. Accomplish the Health Declaration Checklist prior to entry to the University (ThOMedSS for University Staff & Students; StaySafe.PH app for direct-hired staff & University staff dependents).
3. All those with COVID-19 symptoms will NOT be allowed to enter the University premises.
4. Minimum public health standards must be observed at all times while inside the University.
This event is held by the University in coordination with the Department of Health - National Capital Region, Manila City Health Department, and the Catholic Bishops’ Conference of the Philippines (CBCP) as part of the government’s program on rebuilding vaccine confidence and promoting booster doses for COVID-19.
